Irondequoit: Sunday, July 21, 2019. Predeceased by her parents, John & Stephanie Stelmack. Pat is survived by loving family: husband, Robert Hammond; daughters, Marguerite Zorn and Melanie (Dr. Eugene) Toy; grandchildren, Stephanie, Matthew, & Lindsey; stepchildren, Michelle (George) Mirrione & Robert (Christine) Hammond; step- grandchildren, Caitlin, Megan, Kristi, Zachary, & Patrick. The family wishes to thank the Lipson Cancer Center, Lifetime hospice nurses, Kathy, Michelle, Sabrina and Jason for the Loving Care Pat received.

There will be no calling hours. Friends are invited to join her family Thursday, July 25, 11:30 am at St. Kateri At Christ The King Church (445 Kings Highway So.) for her funeral mass.
